The process takes about 15-20 minutes to complete.  Once you start the process, you will need to complete it, or you will have to start the process over. Once the Registration is completed online, print ALL pages. Please take the Athlete Medical Forms (Physical Exam and the Medical Referral Form) to a licensed medical professional to be completed and signed. Once the Medical forms are signed, turn in ALL forms to Kevin Anderson via e-mail or mail to our Post Office Box address.
